New Patient Information for Ot, pt, and/or slp
In order for your child to receive Occupational, Speech, or Physical therapy, we need the following:
- HIPPAA Packet
- Prescription from pediatrician (including ICD-10 diagnosis code and therapy treatment)
Once you have a HIPPA Packet and a prescription for therapy services, your child ready for an evaluation. Contact us to schedule an appointment or come to our office to schedule an evaluation!
New Patient Information for Applied Behavior Analysis
In order for your child to receive ABA therapy, we need the following documents:
- HIPPAA Packet
- Prescription from pediatrician (including ICD-10 diagnosis code and therapy treatment)
- A comprehensive evaluation document, which includes clinical findings, recommendations, and a diagnosis. This can be any of the following:
- Individualized Education Program
- Evaluation by the patient’s school district that has determined eligibility for special education
- Diagnostic evaluation that can be conducted by a Child and Adolescent Psychiatrist or Clinical Psychologist
- A letter from your child’s primary care provider/pediatrician recommending ABA services
- Questionnaire Packet
Once you have all documents, please scan and email to us, or stop by the clinic to drop them off. Once all documents have been received, we can schedule your child for an evaluation by one of our Board-Certified Behavior Analysts, who will create a custom treatment plan for your child.
